Another treatment for erectile dysfunction is a vacuum erection device which is a plastic cylinder that creates a vacuum around the penis resulting in an erection. Penile prosthesis or implant involves an operation that is generally reserved for more difficult cases.

Venous leakage may occur as a result of medical conditions common in older men, such as diabetes, high cholesterol, high blood pressure. Sometimes, venous leakage may occur as a result of anxiety in a sexual situation. This is called "performance anxiety."

Modern treatments for erectile dysfunction abound, and these days treatment professionals often use a combined or holistic approach. Rarely does an erectile dysfunction problem have a single cause. Instead, a mixture of primary physical and secondary psychological factors is responsible. Sexual knowledge, general psychological status, the nature of the relationship, and sexual technique must all be considered, as well as any relevant medical problems or medications.

Erectile dysfunction does not include problems with ejaculation or libido (sex drive), which are completely separate matters. However, a man may experience all or some at once. Loss of libido associated with changes in a man’s testosterone (male hormone) level as he ages is of special significance. But treatment for what appears to be lowered libido is also complex because it, too, may result from several possible factors. These include changes in muscle mass and bone density, rather than libido, and treatments must therefore fit the problems and needs of each man.

But the ability of a man to achieve and maintain erections diminishes with age. Erectile dysfunction, once called "impotence", is the technical term for any problem achieving or maintaining an erection, and, with people living longer, erectile dysfunction is a concern for growing numbers of men and their partners.

What happens during an erection?
An erection is basically about blood flow. When an erection occurs, blood enters the flaccid penis when a neurological signal from the brain via the spinal cord releases a chemical neuro-transmitter. Dilation of arteries follows, and blood is trapped in the erect penis, maintaining an erection. This complex process, called veno-occlusion, relies on a healthy valve effect. A faulty "valve" (called "venous leakage") allows blood to leave the penis too early.
